A listing of the Multifamily Electronic Mortgage Documents for Multifamily Notes, Security Instruments, Guaranties, and Illinois Land Trust documents, as well as certain other miscellaneous forms can be found below. If the Letter of Commitment or accepted Early Rate-Lock Application for a particular transaction requires use of a document not available through this Website and not made a part of the Letter of Commitment or Early Rate-Lock Application, please contact a Freddie Mac Multifamily business representative.

Consult the list of Currently Acceptable Electronic Mortgage Documents [DOC 31K] to determine what documents should be used for your transaction. An Electronic Mortgage Document (i.e., the Multifamily Note) may be provided in multi-state form and in state-specific form for certain states. The state-specific form must be used where applicable. If a document is contained in the list of Currently Acceptable Electronic Mortgage Documents and there is no corresponding specific document file below, the document will be found in the "Miscellaneous Documents" section.
- Select and download the desired Multifamily Electronic Mortgage Document below. Once downloaded, an Electronic Mortgage Document should be completed as appropriate for the particular transaction. Please refer to the Authorized Changes to Electronic Mortgage Documents [DOC 22K] information to determine if any authorized changes to the Electronic Mortgage Documents are required for the transaction.
All proposed modifications to the text of any Electronic Mortgage Document used in connection with a Mortgage loan to be delivered to Freddie Mac for purchase by Freddie Mac must be made in the applicable Modifications Exhibit to the document, except (1) as specifically authorized in the Freddie Mac Multifamily Seller/Servicer Guide, (2) as provided in the Authorized Changes to Electronic Mortgage Documents information, (3) for completing blanks in the document, (4) selecting options as set forth in instructions in the document, and (5) including appropriate signatures, witnesses, and acknowledgements.
